Questionnaires
The Law Firm utilizes various questionnaires designed to provide us with pertinent information about yourself and/or your loved one. The questionnaire also asks you to state your objectives, so that we may be responsive to your needs.
We will customize an Elder Law or Estate Plan based upon the information provided and your objectives. If you are unsure of an answer, please do not guess; merely bring in as much documentation available so that we can assist you in completing the questionnaire.
Information provided at consultations and beyond are governed by the rules of an attorney-client confidentiality, and will be held in the strictest confidence.
